You are submitting an excused absence request for Wind Symphony. You will be emailed a confirmation after submission; keep this for your records. Also keep the physical copy of your documentation. It may be requested by a professor or TA at any point during the semester.

Please note that submitting a request does not automatically excuse your absence. It must be approved by an instructor. Excused absence requests can be submitted at any time during the semester.

2) Documentation for your absence.*
3) The name of your substitute player.

Upload your proof of absence. Absences will not be excused without documentation.
Upload your proof of studio teacher approval. This should follow the general format of "Please excuse [name] from attending Wind Symphony's rehearsal on [date]." An email screenshot is generally easiest.
